The Clinical Patient Navigator ensures service access and meets community care needs through quality assessment, triage, distress screening, service delivery, and care coordination of clinical services between key external constituencies and Sharp. The incumbent meets with all new Radiation Oncology patients at the time of their initial consultation with the Radiation Oncologist. The navigator works closely with the entire Radiation Oncology team of health care providers, namely the Radiation Oncologist, RN, dieticians, radiation therapists, and speech pathologists to create a collaborative plan of care. Individual patient needs are assessed and document via a template in ARIA (a Radiation Oncology Information System or EHR) and/or Cerner based upon the patient's communication needs, financial needs, psycho-social needs, behavioral health needs, and/or transportation needs, etc. The Clinical Patient Navigator also facilitates various cancer patient support groups such as the women's cancer support group (for both English and Spanish-speaking patients), the cancer care giver's support group, children or cancer patient support group, and the men's cancer support group.
Sharp Chula Vista Comprehensive Cancer Center offers a State of the art treatment facility. This technology will encompass an all Varian platform: Two True Beam accelerators, SRS, SBRT, IMRT, Rapid Arc, Respiratory Gating, Brain Lab, ExacTrac, Aria, Eclipse, Varisource, GE Big Bore CT – Simulator all paperless and wireless environment. Our technology is complemented by beautiful and serene gardens, waterfalls and fountains, fireplaces and a healing environment that this Comprehensive Cancer Center offers to our employees, patients and family members alike in line with PlaneTree. Our professional staff consists of Board Certified professionals including: Physicians, Physicists, Certified Medical Dosimetrists, RN’s, Radiation Therapists, a dietician, a speech pathologist, a Patient Navigator, and part-time nurse navigator, as well as front office patient support service staff.
This position is located in Chula Vista
8-hour day shift, M-F no weekends. Variable hours
Required Skills and Qualifications
- Master's Degree in Social Work or Psychology PhD or equivalent
- One year of Social Work experience
- Ability to effectively conduct support group sessions
- Experience providing referrals and resources
- Ability to serve as a liaison to physicians facilitating and coordinating the treatment of patients with cancer
- Bilingual (English/Spanish)
Preferred Skills and Qualifications
- ASW or Licensed Clinical Social Worker (LCSW)
- Experience in Oncology, home health, hospice, or health care/hospital acute care or outpatient setting
