The Medical Social Worker will work with colleagues in the social services department and the treatment teams to complete psychosocial assessments, outline psychosocial needs and resources.  The medical social worker will support and pursue appropriate discharge plans for the patient and family on units assigned or requested to cover at Sharp Chula Vista.  The medical social worker contributes to the development of a comprehensive interventions and safe discharge plan by actively communicating and sharing information with other members of the medical team.  Provides counseling and direct discharge planning services to patients and families utilizing the support of the multidisciplinary team.

The Social Services department provides onsite social services for Sharp Chula VIsta Medical Center. This is a very fast paced position with tremendous opportunity for professional growth within a solid professional Case Management and Social Services Department.  Sharp Chula Vista Medical Center is a 343-bed hospital with the largest array of health care services in San Diego's South Bay.  A Designated Planetree Patient-Centered Care Hospital, Sharp Chula Vista is home to the region's most comprehensive heart program and also offers exceptional services for orthopedic care and women and infants. This position is located in Chula Vista, San Diego County



Part-time; Evening shift with variable start times, will work primarily weekends


Required Skills and Qualifications

  • Master's Degree in Social Work
  • Experience in an acute care, hospital setting
  • Familiarity with unfunded population and complex discharge planning needs
  • Psychiatric disorders, chemical dependency/substance abuse
  • Skills in group facilitation, individual, family and group treatment modalities
  • Excellent communication skills and effective interpersonal communication skills


Preferred Skills and Qualifications

  • LCSW certification
  • One to three years medical social work experience or social work intern experience with Chula Vista population
  • Knowledge of medical terminology and electronic documentation
  • Cerner experience
  • Bilingual (English/Spanish)
