Responsibilities

Summary

The Medical Social Worker is responsible for psychosocial assessment, discharge planning, resource management, supportive counseling and treatment planning with patients and families in the Long Term Care Sub Acute Unit of Sharp Coronado Hospital.  Responsibilities may also include as needed weekend coverage and /or coverage for social work support and discharge planning in the Acute Care Hospital as well.

Sharp Coronado Hospital provides care ranging from emergency services to a nationally recognized total joint replacement program.  The hospital is a 204 bed facility that has a long tradition of providing quality patient centered comprehensive health care.

Sharp Coronado incorporates many personalized aspects into its care philosophy — including patient empowerment, patient and family education, and a physical environment that promotes healing through complementary therapies such as clinical aromatherapy, massage and Healing Touch. Sharp Coronado Hospital is one of the first five hospitals nationally to be designated as a Patient-Centered Hospital by Planetree, an organization committed to personalizing, humanizing, and demystifying the hospital experience for patients and their families.

This position is located in Coronado

 

Hours

Per Diem; 8-hour shift, weekends as needed

 

Required Skills and Qualifications

  • Master's Degree in Social Work
  • One year of hospital or long-term care experience
  • Discharge planning experience
  • Computer fluency
  • Excellent communication, organizational and interpersonal skills

 

Preferred Skills and Qualifications

  • Knowledge of electronic medical records
  • Bilingual (English/Spanish)
