This position will manage coordination of all incoming information via ECIN/On base to intake staff and facility based hospice nurses for hospice referrals. Gathers necessary information and initiates the first steps in establishing a patient billing and clinical record including insurance information and verification, demographic information and enters into Allscripts.
Register patient in Allscripts / Centricity, send to On-base records keeping, compile the admission paperwork for intake staff to process, notify staff of any insurance issues, notify staff of any additional benefit periods if needed, contact other Hospice agencies if patient has been on service with them and obtain admission and discharge info, make all appropriate notations in the notes section of the Allscripts account.
Sharp HospiceCare is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp HospiceCare also provides “pre-hospice” home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia.
Sharp HospiceCare provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa and ParkView Home in Del Cerro, which offers end-of-life patients an alternative to nursing-home placement. The homes — staffed 24 hours a day by licensed nurses — offer a comfortable and supportive environment for up to four patients at one time in each home.
This position is located in La Mesa, San Diego County
FT days: 8:00am-5:00pm, weekends as needed
Required Skills and Qualifications
Three years recent experience in Home Care Intake/Hospice Intake
On-line computer experience in office setting
Basic computer skills. Typing and word processing.
Familiar with medical terminology
Understanding of incoming referrals via ECIN/Onbase
Excellent telephone etiquette. Strong organizational and interpersonal skills
Assist with scheduling for nurses
High school diploma or equivalent preferred
