Responsibilities

Summary
The Clinical Nurse - RN performs assessment, planning, interventions, and evaluation of pain and symptom management and provides care at end of life. The CN-RN collaborates with hospice IDT to ensure effective and safe patient care is implemented. The CN-RN works in highly autonomous environment, and provides nursing and supportive care to patients and their families during end of life. As a member of the Hospice Team, this position will be in a unique position to assist patients and their families clarify goals of care and navigate through the health care system during the end of life journey.
Job responsibilities will include having a thorough understanding of Medicare Conditions of Participation and Local Coverage Determinations, and admitting patients to the hospice program at the appropriate level of care in the patient's preferred environment, providing nursing care and education regarding symptom management and comfort care. This is your opportunity to make a difference for patients at the end of life. In your role as a hospice visiting nurse, you will perform physician assessments, develop the plan of care, obtain physician orders and be the primary resource for your patients. Be able to support the family unit and effectively communicate changes in patient status as they occur.  Ongoing application of the nursing process, through interdisciplinary approach, promoting effective end-of-life palliative care
Sharp Hospice Care is a moderate size Medicare Certified, Joint Commission Accredited organization in an integrated healthcare delivery system. Sharp Hospice Care is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp Hospice Care also provides home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia. Sharp Hospice Care is part of Sharp Grossmont Hospital, nationally recognized as a MAGNET®-designated hospital for patient care and nursing practices.
Sharp Hospice Care provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa, ParkView Home in Del Cerro and BonitaView Home in Bonita which offer end-of-life patients an alternative to nursing home placement. The homes – staffed 24 hours a day by licensed nurses – provide a comfortable and supportive environment for our patients and their families.

This position is located in La Mesa, San Diego County
 
Hours
Day Shift; Week-ends only; Part-time (0.75); 12 hour shift; 8:00 AM – 8:30 PM
 
 
 
Required Skills and Qualifications
 
  • Graduate of Nursing School/Program
  • Current unencumbered California Registered Nurse license
  • Current AHA BLS certification for Healthcare Providers
  • One year current pertinent clinical experience
 
Preferred Skills and Qualifications
  • Bachelors of Science Degree in Nursing
  • Hospice experience
  • Bilingual (English/Spanish)
  • Two years recent acute care hospital RN experience with progressive responsibilities
  • Ability to serve San Diego County
  • Strong nursing skills, excellent time management, strong team player, strong interpersonal and communication skills, flexibility and ability to multi-task
  • Proficient use of the electronic medical record, utilizing keyboarding and computer skills
  • Excellent problem solving skills
  • Fluent knowledge of English, both verbal and written
