The Licensed Vocational Nurse - LVN performs functions and tasks that fully support the patients, families, clinical staff and clinical supervisors. The Licensed Vocational Nurse – LVN will work in the nursing call center as part of a team led by the supervisor.
The nursing call center is staffed by LVN’s in eight hour shifts seven days a week. Patient calls are transferred from the customer service representative to the LVN. The LVN handles each call within their scope of practice providing patient counsel. If the patient need exceeds LVN scope, the call is transferred to the shift RN for triaging. Patient calls received are related to symptom management, changes in status, visit requests and other miscellaneous needs. Patient visit needs are determined and made by the shift RN. Additional job duties will be assigned in support of clinical team. This position offers the opportunity to be cross trained in additional areas, which may include field visits and continuous care.

Required Skills and Qualifications
- High School graduate or equivalent
- Current unencumbered California Licensed Vocational Nurse licensure
- Current BLS certification for Healthcare Providers issued by the AHA
- 2-3 years recent LVN experience, preferably in an acute care hospital setting
- Strong computer skills and efficient use of EMR
- Knowledge of nursing process.
- Effective interpersonal and communication skills.
- Fluent knowledge of English, both verbal and written
Preferred Skills and Qualifications
- Bilingual English/Spanish
- Two years recent Hospice experience
- Acute care experience
- Strong team work, problem solving and critical thinking
- Efficient time management skills, flexibility
