Summary

The Licensed Vocational Nurse - LVN performs functions and tasks that fully support the patients, families, clinical staff and clinical supervisors.  The Licensed Vocational Nurse – LVN will work in the nursing call center as part of a team led by the supervisor.
The nursing call center is staffed by LVN’s in eight hour shifts seven days a week. Patient calls are transferred from the customer service representative to the LVN. The LVN handles each call within their scope of practice providing patient counsel. If the patient need exceeds LVN scope, the call is transferred to the shift RN for triaging. Patient calls received are related to symptom management, changes in status, visit requests and other miscellaneous needs. Patient visit needs are determined and made by the shift RN. Additional job duties will be assigned in support of clinical team. This position offers the opportunity to be cross trained in additional areas, which may include field visits and continuous care.

Services range from relief of physical discomfort to emotional and spiritual support. Sharp Hospice Care also provides home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia. Sharp Hospice Care is part of Sharp Grossmont Hospital, nationally recognized as a MAGNET®-designated hospital for patient care and nursing practices.
Sharp Hospice Care provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa, ParkView Home in Del Cerro and BonitaView Home in Bonita which offer end-of-life patients an alternative to nursing home placement. The homes – staffed 24 hours a day by licensed nurses – provide a comfortable and supportive environment for our patients and their families.
Location
This position is located in La Mesa, San Diego County.
 
LVN- Nursing Call Center
 
Hours: Per Diem position - various 8-hour shifts, working every other weekend
 
Required Skills and Qualifications
  • Current unencumbered California Licensed Vocational Nurse licensure 
  • Current BLS certification for Healthcare Providers issued by the AHA
  • Valid California Driver's License - Utilizes reliable transportation and possesses adequate personal insurance coverage.
  • Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines. 
  • 2-3 years recent LVN experience, preferably in an acute care hospital setting
  • Strong computer skills and efficient use of EMR
  • Effective interpersonal and communication skills.
  • High School Diploma
 
Preferred Skills and Qualifications
  • CHPN
  • Bilingual English/Spanish
  • 2 years recent Hospice experience
  • Acute care experience
  • Strong team work, problem solving and critical thinking 
  • Efficient time management skills, flexibility
