Responsibilities

Summary

Under the direction of the hospice medical staff, the Nurse Practitioner will provide home/SNF/RCFE based clinical assessment of hospice patients to determine eligibility under the Medicare Hospice Benefit criteria. The Nurse Practitioner will incorporate evidence based clinical data to determine and support eligibility criteria and document the same in the hospice EMR. The Nurse Practitioner will provide ongoing communication and collaboration with the hospice Interdisciplinary Team regarding findings/assessments/ and plan of care. The Nurse Practitioner will provide education and consultation to staff and provide consultative clinical assessment of hospice and/or palliative care patients.
Sharp HospiceCare is a moderate size Medicare Certified Joint Commission Accredited integrated healthcare delivery system.
Sharp Hospice Care is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp Hospice Care also provides “pre-hospice” home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia.
Sharp Hospice Care provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa, ParkView Home in Del Cerro, and BonitaView Home in Bonita, which offer end-of-life patients an alternative to nursing home placement. The homes – staffed 24 hours a day by licensed nurses – provide a comfortable and supportive environment for four – six patients at one time in each home.

Location
This position is located in La Mesa, CA; requires travel within San Diego County
 
Hours
8 hour shift, Full-time, 7:00am - 4:30pm, working weekends as needed 
 
Required Skills and Qualifications
  • Masters of Science Degree in Nursing or related Health Care degree or equivalent experience required
  • Current unencumbered California Registered Nurse licensure
  • Current unencumbered California Nurse Practitioner licensure
  • Current BLS certification for Healthcare Providers
  • Expert level computer skills
  • Strong advanced nursing skills
  • Excellent time management, strong critical thinking, strong communication and strong physician collaboration skills
  • Desire to work with end-of-life care
  • Speak and understand English well, both written and verbal
 
Preferred Skills and Qualifications
  • One year experience
  • Hospice and Geriatrics experience
  • Acute Care Nurse Practitioner Adult or Adult Gero (AACNPC or ACNPC-AG)
  • Advanced Oncology Certified Nurse Practitioner (AOCNP)
  • AANPCB Adult Nurse Practitioner (AANP – accredited)
  • Adult Care Nurse Practitioner – Board Certified (ACNP-BC)
  • Adult-Gerontology Acute Care Nurse Practitioner-Board Certified (AGACNP-BC)
  • AANPCB Adult Gerontology Primary Care Nurse Practitioner (AGNP)
  • Adult Nurse Practitioner – Board Certified (ANP-BC)
  • AANPCB Family Nurse Practitioner (FNP)
  • Adult-Gerontology Primary Care Nurse Practitioner – Board Cert (AGPCNP-BC)
  • Family Nurse Practitioner – Board Certified (FNP-BC)
  • DEA Controlled Substance Registration
  • Bilingual (English/Spanish)
