To provide consultation, psychosocial counseling and discharge planning support services to the Sharp Health Care patients and families. Refers to internal resources of hospital that can assist patients and families. Refers basic community resources Facilitates the timely discharge of patients.. Provides refers patient/family to them ongoing assistance after leaving.
The Intensive Treatment Program (ITP) is a 14 bed locked 24-hour care unit and part of 46 beds Inpatient Psychiatric Unit. This unit is designed for adults who are gravely disabled or so impaired in their psychological functioning as to constitute a danger to themselves or others. Highly focused efforts are provided by the Multidisciplinary Team to assess and stabilize persons in a safe, caring and therapeutic environment.
The Medical Psychiatric Program (MPP) is a 12 bed open unit and part of 46 beds Inpatient Psychiatric Unit. This unit is designed for adults who have both medical and psychological diagnoses requiring 24-hour skilled nursing inpatient assistance. The unique strength of this program comes from combining physical and psychological care in one setting and best serve the needs of persons with a primary psychiatric diagnosis, who also have additional medical complications.
The Older Adult Program (OAP) provides inpatient, short-term comprehensive care for patients 55 years of age and older experiencing acute psychiatric disorders, cognitive impairment, and age-related physiological difficulties. Each patient receives care from an interdisciplinary team of Behavioral Health professionals. Treatment focuses on restoring function and allowing older adult patients to become as independent and productive as possible.

Required Skills and Qualifications

- Current BLS certification for Healthcare Providers
