Supervisory responsibility for the Social Service Department at Sharp HospiceCare ensuring the delivery of quality patient care and the efficient financial management of budgeted operations. This position will be responsible for the social workers for Sharp Hospice, bereavement counselors and music therapists. Oversees, implements and supervises the programs and services of the following counseling departments: bereavement, social work, and spiritual care. Participates as a member of the hospice management team.
Sharp HospiceCare is dedicated to providing comprehensive care and compassionate support to individuals facing life-limiting illnesses, along with their families and friends. Services range from relief of physical discomfort to emotional and spiritual support. Sharp HospiceCare also provides “pre-hospice” home based palliative care consultation for patients with advanced illness such as heart failure, chronic obstructive pulmonary disease and dementia.
Sharp HospiceCare provides in-home services and also provides the region’s first freestanding hospice homes, LakeView Home in La Mesa and ParkView Home in Del Cerro, which offers end-of-life patients an alternative to nursing-home placement. The homes — staffed 24 hours a day by licensed nurses — offer a comfortable and supportive environment for up to four patients at one time in each home.
The social service department provides end of life counseling and resource services to the hospice patients and their family members. All social workers are master prepared and specialize in the area of death and dying. The department consists of 14 medical social workers who work closely within an interdisciplinary model to provide a holistic approach to end of life care. Social Workers utilize a case management style for their caseloads, working independently in an environment that offers ample support. The department offers group and individual supervision to help promote continued professional development for our staff. All clinicians are provided cell phones and lap tops that are connected to the main computers.
This position is located in La Mesa, San Diego County.
Full-time; Typically scheduled 8:00 am - 5:00 pm including weekends as needed. Hours may fluctuate depending on needs.
Required Skills and Qualifications
- Master's Degree in Social Work
- Three years' direct hospice experience
- Three years' supervisory experience
- Current BLS from the American Heart Association required
