What You Will Do
Assures that optical imaging functions are completed accurately and in a timely manner. General responsibilities include prepping, indexing and scanning of medical records for both current and discharged patients. Position also provides support for other HIM functions such as customer service (i.e. reception, record retrieval for patient care).
- H.S. Diploma or Equivalent
- 1 Year Health Information Management
Observes and respects the confidentiality of information in regard to patients, physicians and fellow employees as stated in the policy manual.
Follows established guidelines for release of information.
- Customer service
Provides general clerical support for the department as needed: answering telephone, retrieving records for physician completion and faxing information.
Review scanned documents comparing the scanned image to the original document to assure clarity and accuracy prior to saving in EDM product.
As images are reviewed, correct rotation, delete duplicates, rescan skewed or poor quality images as needed and index each document type.
Number of pages per document type should be compared to the total number scanned.
Transfer paper record in ®Cerner Profile application to appropriate location for further QC activity by the Optical Imaging Specialist II.
- Record processing
Remove all staples prior to starting process.
Repair documents, if needed (tape torn pages).
Tape down any sheets less than 8.5" x 11".
Tape the top of rhythm strips so they do not catch on feeder.
Arrange multipage documents in date order per department guidelines.
Review documents for appropriate patient identification.
Affix patient name, EMRN and account number on both sides of the document.
If document is likely to result in poor image quality once scanned, affix "poor quality original" stamp prior to scanning.
Print and insert appropriate bar code sheets for each document type.
Perform daily maintenance of the scanner.
Calibrate the scanner for proper image quality, as needed.
COLD fed technology will be used when available, i.e. lab, radiology, pathology, PICIS, MAC lab and Clinicomp.
All other paper documents are optically scanned and stored, i.e. progress notes, physician orders, etc.
'Fan' the stack of documents to assure staples have been removed and there are no pages sticking together; then scan.
Ensure that all pages are in the same proper orientation (rotate as needed).
Review scanned documents comparing the scanned image to the original document page by page, to assure clarity and accuracy prior to indexing in the EDM product.
Verify correct document type; correct patient name and correct visit number then index.
- Record requests
Researches and retrieves medical records for requests from the nursing units and/or physicians as needed.
- Transcription reports
Assures that transcription reports without appropriate demographic information is researched for the correct information and faxed to transcription service immediately upon receipt.
Knowledge, Skills, and Abilities
- Optical imaging knowledge, preferred.
