The Advanced Illness Management (AIM) Navigator’s role is to use the nursing process to determine the specialty needs of people living with serious advancing illness and facilitate the development of a care plan and interventions aligning patient goals, preferences and values with care received. The primary focus of the AIM Navigator will be advance care planning discussions, goals of care discussions and hospice enrollment in the ED.
The AIM Navigator is an expert clinical practitioner, using an evidence-based approach, in collaboration with key stakeholders to ensure safe and effective care and care coordination. The AIM Navigator partners with care team members to coordinate care and resources and will ensure transitions of care are realistic, safe and attainable. Responsibilities include participating in data gathering, analysis and providing recommendations for improving overall quality of care based on outcomes. The AIM Navigator serves as an expert resource, educator and role model in the care and management of those living with serious advancing illness. He/she will function as a member of the Interdisciplinary Team coordinating care with various team members.

This position is located in Serra Mesa, CA (San Diego County)
Day shift; Part time (.6); 10 hour shifts; every other weekend; Shift start time and end time variable; specifics regarding scheduling, including holiday schedules, to be discussed during the interview
Required Skills and Qualifications
Current unencumbered California RN license
Current AHA BLS certification for Healthcare Providers
BSN degree; Equivalent clinical experience my substitute for BSN
Minimum of 5 years of nursing experience plus 3 years clinical experience in geriatrics, palliative care, end of life care or similar clinical experience
Preferred Skills and Qualifications
Certified Hospice and Palliative Care Nurse (CHPN) preferred and CHPN certification preferred or obtained within 2 years of placement in new role
Clinical expertise in the field of geriatrics, palliative care and end of life care
Demonstrates knowledge and application of evidence-based clinical principles, clinical guidelines and practice standards in the care and management of people living with serious advancing illness
Skilled communicator with demonstrated critical thinking and problem skills
Able to educate and mentor others
Demonstrates effective time management, prioritization and documentation skills
Proficient with Information Systems and computer programs
