What You Will Do
Day-to-day clerical support for the functional areas for SMBHW
Required Qualifications
- H.S. Diploma or Equivalent
Preferred Qualifications
- 1 Year Medical record experience
Essential Functions
- Birth certificates registration
Maintains current birth registry log to assure appropriate follow up.
Maintains inventory log for vital statistics.
Prepares a correct and legible birth certificate with all items completed.
Provide the certificate to the parent/informant for review and signature.
Obtain the certifier/attendant's signature, after both the public portion and the confidential medical and social information items have been entered on the form.
Certificates are registered with the local registrar with ten (10) days of the birth.
All birth certificate amendments are completed and submitted within ten (10) days of the request.
If the correction is a hospital error, the clerks will assist the parent(s) in applying for the amendment. - Communication
Birth registry clerk will prepare a complimentary birth certificate for each birth and present to the parent(s) upon discharge.
Birth registry clerk will inform parents of the Paternity Opportunity Program, a voluntary paternity acknowledgement process required by federal and state law.
Clerks will provide information about the program and if necessary assist with the completion of the forms.
Completed forms will be forwarded to the local Registrar of Vital Statistics within ten (10) days of the birth.
Provides information to parent(s) regarding other available programs, i.e. NANA, Baby Breath.
To assist the parent(s), the birth clerk will complete the required forms. - Confidentiality
Observes and respects the confidentiality of information in regard to patients, physician and fellow employees as stated in the policy manual.
Follow established guidelines for release of information. - Perform general clerical duties
Assists with general clerical support to HIM department when needed.
