Responsibilities

Summary
The Care Specialist Population Health provides support to administrative team by coordinating Population Health priorities. Ensures effective and efficient systems. Ensures processes are utilized to achieve and improve patient outreach, care and operational outcomes. Supports and facilitates care coordination activities and services provided by RN Case Managers and assists with oversight of Care at Home activities and initiatives. Collaborates and assists with strategies that target care management engagement and growth.
This Care Specialist position is with the Population Health Department of Sharp Rees Stealy (SRS).  The Population Health Department includes the Ambulatory, Disease Management and Diabetes Management programs.  The department employs case managers who telephonically manage SRS patients with cardiac disease, diabetes, COPD or asthma whose current status is below optimum.  The team includes, but is not limited to Case Managers, Diabetes Specialist Educators and support staff.

Location
This position is located in Kearny Mesa. Occasional travel between Sharp HealthCare offices; must provide own transportation
 
Hours
Full-time, Days (7:00 A.M. to 3:30 P.M.) Temporary Position 
 
Required Skills and Qualifications
  • High School graduate or equivalent required
  • Two years’ minimum work experience preferably in a managed care setting
  • Evidence of strong mathematical skills, strong analytical skills, and the ability to work independently to organize and accurately execute large volumes of tasks with a minimum of supervision
  • Evidence of excellent engagement and customer service skills
  • Excellent typing skills and proven expertise generating documents and reports utilizing software applications including but not limited to Microsoft PowerPoint, Excel and Word
Preferred Skills and Qualifications 
  • Two years college coursework
  • Medical Assistant education
  • Knowledge of medical office patient navigation, organization, and scheduling models
  • Bilingual in English and Spanish
