The Clinical Case Manager will be responsible for coordination of services utilizing a multidisciplinary plan to ensure the effective and efficient consumption of available health care resources while maintaining quality and cost-effective care for patients.  The Clinical Case Manager facilitates and coordinates services with health care professionals and monitors the patient’s progress towards the achievement of quality and cost outcomes.  The Clinical Case Manager is an expert clinical practitioner who participates in the development and evaluation of clinical guidelines and standards of care.
Responsibilities include accurately completing a patient assessment, coordinating an interdisciplinary plan of care and tracking patient care and services throughout the care continuum, collecting data and assisting in trending and analysis of aggregate service line data to be used in identifying best practices and performance improvement opportunities.   He/she will be responsible for planning/coordinating community education and outreach programs.  The Clinical Case Manager is self-directed, motivated and must be able to work independently.
The Clinical Case Manager position is part of the Satellite Senior Health Centers.  The Sharp Senior Health Centers are licensed outpatient departments of SMMC serving the unique healthcare needs of Seniors 65 years of age and older.
This position is located in San Diego, CA.  The Clinical Case Manager will primarily work at the Downtown Sharp Senior Health Center.
8-hour day shift, 8:00 am - 4:30 pm; no weekends
Part-time (0.8 FTE) position 64 hours per pay period
Required Skills and Qualifications
  • Current unencumbered California Registered Nurse licensure
  • Current BLS certification for Healthcare Providers
  • Bachelor of Science in Nursing or related health care degree
  • 5 years acute care nursing experience
  • Expert clinical practitioner with advanced clinical skills and in-depth knowledge of gerontology 


Preferred Skills and Qualifications
  • Ambulatory care experience
  • Understanding of performance improvements and research methodologies
  • Excellent communication skills with the ability to interact with all customers.
  • Assertive and articulate in difficult situations
  • Skilled in conflict management and resolution
  • Self-directed, autonomous and able to set and achieve priorities
