Responsibilities

Summary
The Community Health Worker (CHW) will work telephonically and  in the community with the English and Spanish speaking population to assist with culturally and linguistically appropriate care and to reduce actual and/or perceived health care disparities among the patient population at Sharp. The CHW will work closely with medical providers, primary care teams, and other agencies in a collaborative fashion to improve patient care and outcomes.  The CHW must be a team player and establish positive, supportive relationships.  He/she must work effectively with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions. The CHW will act as a resource or representative, helping patients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.  He/she will assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
Population Health is a multidisciplinary department of over 65 staff members working collaboratively across the Continuum of Care to provide outreach, education and connections with the chronically ill population within SRS in order to improve health, improve quality of care and decrease admissions, readmissions and emergency department visits.

Today, Sharp Rees-Stealy has 22 locations (and still growing) and over 500 primary care doctors and specialists representing virtually every field of medicine.



Location
This position is located in Kearny Mesa, San Diego County.
 
Hours
Per diem: 9-hour day shift; variable start and end times. 

Required Skills and Qualifications
  • Any combination of 2 years health/social services experience and/or education.  
  • Verifiable good driving record and reliable transportation.  
  • Bilingual/bicultural (Spanish).
  • Driver's license required.

Preferred Skills and Qualifications

  • Medical Assistant training preferred.
