What You Will Do
Organizes and coordinates daily intake/referral processes for the Home Health agency to ensure that the initiation of care is timely and according to physician orders and within regulatory guidelines. Acts as the key linkage between hospital case management services, medical group physicians and office staff, hospice agencies and Home Infusion services. Responsible for the coordination of complex patient issues related to home care services.
- H.S. Diploma or Equivalent
- 1 Year experience in a healthcare organization.
- California Pharmacy Technician License (TCH) - CA State Board of Pharmacy -REQUIRED
- 1 Year Home Health agency.
- Referral Management
Uses referral management software to determine appropriateness of services.
Completion of service requests through nurse visit or delivery.
Follow up with physicians for treatment orders.
Informs patient/families of scheduled initial visit.
Call each referral to confirm demographic information and primary physician information.
Establishes courteous rapport with patients.
Exhibits flexibility and adaptability .
Works with billing department for insurance verification and authorizations.
Verifies patient's insurance eligibility and coverage.
Communicates with referral sources and hospital case mangers regarding insurance co-pays.
Notifies patient and caregiver of their cost share.
Communicates with the billing department regarding patient information and authorization.
Patients are screened for ancillary needs.
All inquiries are screened and converted to home health referrals based on agency admission criteria.
- Clinical IV Practice
Displays consistently high accuracy in the preparation of IV admixtures, organize workload to accomplish all admixture tasks, equipment maintenance and monitoring, maintain documentation, up to date process validation per department procedures.
Inventory Management including process order entry in EMR (Electronic Medical Record), collaborate with department buyers, and coordinate with other disciplines as appropriate.
- Other Duties
Additional responsibilities as necessary per department’s needs.

- Must have skills and experience with handling difficult situations. Applying empathy, courtesy and compassion.
- Must have skills and experience with different approaches when working with co-workers, staff and the public.
- Must be flexible and able to assume weekend rotations.
- Must have basic knowledge of insurance.
- Must be willing to train.
