Responsibilities

Summary
The Intake Coordinator-LVN organizes and coordinates daily intake/referral processes for the home health agency to ensure that the initiation of care is timely and according to physician order and within regulatory guidelines.  The Intake Coordinator acts as the key linkage between hospital case management services, medical group physicians and office staff, hospice agencies and home infusion services.  The Intake Coordinator is responsible for the coordination of complex patient issues related to home care services.
The Intake Coordinator assists in the timely facilitation and coordination of home health services for patients transitioning from the acute or sub-acute setting. Upon determination that the patient requires home care, the Intake Coordinator accepts patient via electronic referral management system or telephonic means, screens patient for eligibility criteria, assesses patient’s physical, mental, and psycho-social post-acute care needs and homebound status, and documents findings in the clinical record. The Intake Coordinator assists in establishing a definitive home care plan by communicating with physicians for orders and/or services. The Intake Coordinator coordinates complicated discharges with case managers, communicates frequently with Intake Supervisor/staff regarding patient types and corresponding agency capacity and evaluates reimbursement sources for thoroughness and accuracy. The Intake Coordinator proactively works to solve problems and seek resolution and provides support to the organizational structure to achieve desired operational outcomes.

Location
This position is located in Kearny Mesa, San Diego County
 
Hours
8 hour shift, Part-time, 8:00 - 5:00pm, working weekends as needed
 
Required Skills and Qualifications
  • Graduate of an accredited Licensed Vocational Nurse (LVN) School of Nursing
  • Current unencumbered CA Licensed Vocational licensure
  • Must have 1-2 years home health experience
  • Must have knowledge of overall home health management including admission criteria, nursing process and insurance limitations
  • Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations
  • Possesses the ability to use and apply clinical knowledge to the referral process.
  • Effective interpersonal communication skills
  • Fluent knowledge of English, both verbal and written
  • Ability to demonstrate compassion and courtesy when dealing with difficult situations
  • Capable of working under pressure and time constraints including overtime to complete daily assignments.
  • Use of sound judgement and tactful approach when dealing with co-workers, staff and the public
 
Preferred Skills and Qualifications
  • Home Health or Hospice experience
  • Home Health Liaison experience
  • Community Outreach Coordinator experience
  • Computer skills
  • Management experience
  • Bilingual (English/Spanish)
