The Intake Coordinator organizes and coordinates daily intake/referral processes for the home health agency to ensure that the initiation of care is timely and according to physician orders and within regulatory guidelines. The Intake Coordinator acts as the key linkage between hospital case management services, medical group physicians and office staff, hospice agencies and home infusion services. The Intake Coordinator is responsible for the coordination of complex patient issues related to home care services.
The Intake Coordinator assists in the timely facilitation and coordination of home health services for patients transitioning from the acute or sub-acute setting. Upon determination that the patient requires home care, the Intake Coordinator accepts patient via electronic referral management system or telephonic means, screens patient for eligibility criteria, assesses patient’s physical, mental, and psycho-social post-acute care needs and homebound status, and documents findings in the clinical record. The Intake Coordinator assists in establishing a definitive home care plan by communicating with physicians for orders and/or services. The Intake Coordinator coordinates complicated discharges with case managers, communicates frequently with Intake Supervisor/staff regarding patient types and corresponding agency capacity and evaluates reimbursement sources for thoroughness and accuracy. The Intake Coordinator proactively works to solve problems and seek resolution and provides support to the organizational structure to achieve desired operational outcomes.

This position is located in Kearny Mesa, San Diego County
8 hour shift, Full-time, 8:00am - 5:00pm, working weekends as needed
Required Skills and Qualifications
- Current unencumbered CA Licensed Vocational licensure
Must have experience with handling difficult situations applying empathy, courtesy and compassion.
Must have experience with using different approaches when working with co-workers, staff and the public.
Must assume a position in the weekend rotations.
Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations
Preferred Skills and Qualifications
- One year Home Health experience
- Experience as a Home Health Liaison or Community Outreach Coordinator
- Knowledge and experience of overall home health management including admission criteria, nursing process, insurance limitations and referral process.
- Bilingual (English/Spanish)
