The Medical Social Worker  is a skilled communicator and is able to provide psychosocial support services to a population of individuals, some elderly with depression or grief/loss issues, and mobilize patient and family resources, both personal and environmental.  The MSW will support team-based Shared Action Planning to achieve maximum benefit from the Plan of Care.  She/he possesses and applies knowledge of the health care system, the community, and/or state and federal programs to support patients, their families, and caregivers to obtain optimum care, experience successful self-management opportunities, and restore health.  The Medical Social Worker  is an expert communicator, interfacing frequently with the staff nurses and leads within the CCN/CCM/DM departments and will monitor outcomes for ongoing process improvement.  

The Social Services Department provides a wide range of services to patients and their families.  The department consists of Social Workers, Discharge Planners, and social work interns.  Social Workers are part of the treatment team and work in collaboration with nurses, pharmacists, physicians to ensure that every patient receives the best care possible.

This position is located in Downtown San Diego



Per Diem -Variable hours


Required Skills and Qualifications

  • Master's degree in Social Work
  • Intermediate knowledge of Microsoft Excel, Word, and Outlook



Preferred Skills and Qualifications

  • LCSW licensure
  • Outpatient experience in a managed care environment
  • Bilingual (English/Spanish/Tagalog)
