What You Will Do
Under the direction of the Manager of Process Improvement and Practice Transformation and the Medical Director of Population Health, this position is accountable for providing support and training to physician practices to execute on opportunities for improved quality, clinical workflows, and performance. This role is responsible for maintaining collaborative team relationships with peers, colleagues, and physician practices to effectively and efficiently achieve organizational goals. Supports process improvement to enhance physician and staff workflows and drive performance improvement.

Required Qualifications

  • Bachelor's Degree in health related field or organizational leadership.
  • 3 Years Experience in health care.
  • Progressive responsibility in the application of process improvement principles, implementation of quality improvement methodologies, project management, and team facilitation.
  • Experience in working with cross functioning teams and building relationships with clinical and non-clinical personnel.
  • Experience using verbal and written communication to facilitate and bring teams to consensus.
  • Experience working with physicians.
  • Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Drive Guidelines.
  • Valid CA Drivers License

Other Qualification Requirements

  • Six Sigma Yellow Belt Certification is required within 6 months of hire. The department is responsible for tracking and maintaining this certifications. This position is required to travel between Sharp facilities and SCMG provider offices; Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Drive Guidelines.

Essential Functions

  • Data analysis
    Interpret, analyze data, and provide the information in a format that is actionable for providers.
    Utilize data to benchmark and evaluate outcomes of quality and process improvement techniques.
    Report on quality and process improvement initiatives and measures. Coordinate collection, analysis, and write up of evaluation of practice improvement efforts, with significant attention to detail and accuracy.
    Completes medical record reviews, compares current practice against best practice guidelines and provides recommendations to improve patient care.
  • Leadership
    Manages a number of physician practices to facilitate successful delivery of performance improvement solutions under the direction of the Manager of Performance Improvement and Practice Transformation.
    Maintain a high degree of independence, flexibility, initiative and commitment.
    Work effectively with diverse population both internally and externally.
    Maintains awareness of and notifies leadership of actual or potential risk situations.
  • Quality and process improvement
    Under the oversight of the Manager of Performance Improvement and Practice Transformation, work with SCMG Physicians and practices to develop focused performance improvement plans to progress quality measures.
    Identify opportunities for quality metric improvement, makes recommendations for change, and leads the implementation of best practices to meet established goals.
    Analyze problems, devise and implement an appropriate improvement plan in clinical and business practices using a variety of documented approaches.
    Facilitate process redesign efforts; support their achievement of improved patient outcomes, increased patient, provider and health care team satisfaction, and efficiency.
    Solve process problems crossing multiple functional areas.
    Effectively present information and respond to questions.
    Manage competing objectives and prioritize accordingly while using strong business judgment.
    Meets individually with providers and staff to assist with problem solving, recommend, and facilitate improvement strategies.
  • Teamwork and development
    Team Building and Group Facilitation
    Promotes cross departmental communication, collaboration, and performance improvement as a means of achieving cost effective, high quality care.
    Effectively facilitate team meetings between all levels of staff and function as a positive liaison between staff, practice and med group leadership in support of program goals and deliverables.
    Maintains collaborative team relationships with peers, colleagues, and physician practices to effectively and efficiently achieve goals.
    Motivate practice leadership to adopt new ideas, methods, and processes that align with SCMG strategic goals.
  • Training and education
    Serve as an educator, communicator, and resource to physicians, providers and staff to understand care management and quality improvement.
    Coordinate training opportunities related to quality and process improvement for practices.
    Organize and adapt training materials for diverse learners.
    Educate and train practice staff in quality and process improvement techniques and methodologies.
    Educate and introduce foundational concepts and key principles of lean Six Sigma to physician practices.

Knowledge, Skills, and Abilities

  • Knowledge of change management principles, methodologies and tools required.
  • Must be proficient in Microsoft Office applications.
