This position is located in Serra Mesa, San Diego County
10 hours variable shift, weekends as needed.
Required Skills and Qualifications
- Graduate of an accredited college of Pharmacy
- Current unencumbered CA Pharmacist license
- Current CPR certification
- Minimum of one (1) year current Pharmacist experience in a hospital pharmacy environment
- Pharmacy Residency (PGY1) with experience in Transitions of Care.
- Computer skills are highly desirable, especially in the application of word processing, spreadsheet, and database creation and management.
Preferred Skills and Qualifications
- Doctor of Pharmacy (Pharm.D)
- Completion of a PGY2 Transitional Care residency
- Board Certified Ambulatory Care Pharmacist (BCACP) and/or equivalent Board Certification (BCPS) preferred.
The Pharmacist works with the Care Transition Coach who manages all activities of the Community-Based Care Transitions Program (CCTP). The pharmacist applies clinical expertise in evidence-based practice within the case management and care transitions specialty area and functions in the following roles: coach, clinical expert, educator, researcher, and consultant. The position will function in comprehensive medication management, providing expertise to patients, families, physicians, and other members of the care team as to how to use medications to achieve clinical goals. Position will work solely with patients who are defined as the target population in the program design both in the acute hospital setting and in their homes to help manage their chronic conditions outside of the hospital setting.
The Pharmacist will also rotate through shifts in the main pharmacy. The shifts will consist of both distribution and clinical elements.
