This position is located in La Mesa, San Diego County. The work location will be Sharp Grossmont Hospital.
THIS IS A TEMPORARY POSITION
8 hours day shift, 8:00 AM – 4:30 PM, no weekends.
Required Skills and Qualifications
- Doctor of Pharmacy (Pharm.D.)
- Current unencumbered CA Pharmacist license
- Completed of a Pharmacy Practice Residency, with experience in Transitions of Care
- Minimum of one (1) year current experience in a hospital pharmacy environment or equivalent healthcare setting.
Preferred Skills and Qualifications
- Completion of a PGY-2 Transitional Care residency program
- Board Certified Ambulatory Care Pharmacist (BCACP) and/or equivalent Board Certification (BCPS)
- Bilingual (English/Spanish)
THIS IS A TEMPORARY POSITION.
The Pharmacist applies clinical expertise in evidence-based practice within the case management and care transitions specialty area and functions in the following roles: coach, clinical expert, educator, researcher, and consultant. The position will function in comprehensive medication management, providing expertise to patients, families, physicians, and other members of the care team as to how to use medications to achieve clinical goals. Position will include overall Comprehensive Medication Management program coordination; establishing a patient referral process, oversight of deployment of referral services to patients, education for patients/families on the program, monitoring of clinical information, collaborating on establishing/changing care plans as appropriate to scope of practice, making recommendations to MDs on clinical interventions and overall data collection/reporting for program. Position will work solely with patients who are defined as the target population in the program design both in the acute hospital setting and in their homes to help manage their chronic conditions outside of the hospital setting.
Position will manage populations of patients who meet high risk criteria in order to improve disease state outcomes and improve medication related safety. Insures safe and cost effective medication therapy implemented for all patients in their care and monitor care appropriately. Provide continuity of care for medications during transition of care and discharge services as appropriate. This position will require leadership skills (system thinker and change agent). The pharmacists will be required to work with physicians, inpatient nurses, outpatient pharmacists, home health nurses, case management, social workers, and others as relevant to achieving clinical goals that are important to the patient and family. The pharmacist will ensure appropriate screening and interventions on patients and documentation of transitional care activities. The pharmacist will serve as the pharmacy department liaison with the identified medical division and committees for their area of specialty. The pharmacist will demonstrate the ability to perform competencies as outlined in Pharmacy Competency Assessment tools. Serve on committees as directed by the Director of Pharmacy. Other responsibilities will include development of discharge program to transition patients out of the acute care transitions phase and return to self-care, coordinating education programs for patients in program, integrating documentation/outcomes into patient’s Sharp HealthCare medical record, coordination of resources or making referrals for patients to include physician office visits, financial assistance, social services, etc. and data collection, evaluation of program’s effectiveness and other duties as assigned.
The Community-based Care Transitions Program (CCTP), created by Section 3026 of the Affordable Care Act, tests models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries.
This is a float pool position through Staffing Resource Network (SRN). The SRN department offers opportunities to experience a wide range of patient care at various sites within Sharp HealthCare.
