Responsibilities

Summary
As part of a multidisciplinary team, the Medication Therapy Management Pharmacist will conduct comprehensive medication reviews for appropriateness of drug therapy, therapeutic duplications, drug interactions, gaps in therapeutic care, etc. pertaining to health plan formulary, quality metrics, and clinical practice guidelines. He/she will review patient medication profiles relating to outpatient anticoagulation therapy, post-discharge reconciliation, chronic and complex disease states, and population health. The clinical pharmacist may also participate in advisory groups, conduct prior authorization and utilization review, and develop pharmacy clinical programs to promote safe and cost-effective benefits to patients.

Location 

This position is located in Kearny Mesa, San Diego County

 
Hours
FULL-TIME

8 hour day shifts; 8:00 am - 5:00 pm; weekends as needed; on-call rotation.

 
Required Skills and Qualifications
  • Current unencumbered CA Pharmacist license
  • Minimum of one (1) year experience in a managed care or ambulatory care setting.
  • Certification in anti-coagulation, geriatric pharmacy, or medication therapy management obtained within 1 year of hire
  • Demonstrates proficiency in clinical skills, clinical judgment and leadership
  • Experience in anticoagulation therapy patient monitoring
  • Working knowledge of Microsoft Office software programs
 
Preferred Skills and Qualifications
  • Doctor of Pharmacy (Pharm D)
  • PGY in Managed Care preferred
  • Board Certified Ambulatory Care Pharmacist (BCACP) or equivalent Board Certification (BCPS)
  • Experience with anti-coagulation and/or chronic disease clinics preferred
  • Current one (1) year of Pharmacist experience in health plan, medical group, or PBM pharmacy environment or ambulatory clinical setting
  • Bilingual (English/Spanish)
