The Safety Medication Practice Pharmacist performs activities related to Adverse Drug Event (ADE) surveillance and develops, implements, and monitors control programs designed to minimize the incidence of patient ADEs and enhance patient safety.
The SMP Pharmacist will lead the institution’s medication safety surveillance and improvement efforts, promoting our “culture of safety”. This key hospital pharmacy representative co-chairs the Safe Medication Practice Committee with a physician, working with pharmacy, nursing, quality, medical, and information systems staff to improve the medication process and outcomes for Sharp Mesa Vista Hospital. This individual will analyze the site’s medication safety events, leading the identification of prevention opportunities. This site SMP Pharmacist will regularly participate in the System Safety Committee and collaborate with his or her SMP Pharmacist counterparts from all Sharp facilities, sharing best practices and performing formal event classification inter-rate reliability testing. The SMP Pharmacist will succeed through enthusiastic collaboration, communication, training, and ongoing process improvement initiatives.

This position is located in Serra Mesa, San Diego County.
8 hours variable shift, weekends as needed.
Required Skills and Qualifications
Doctor of Pharmacy (Pharm.D) preferred
Current unencumbered CA Pharmacist license
Current BLS certification for Healthcare Providers
- Minimum of three (3) years current Pharmacist experience or completion of PGY1 or PGY2 residency in a hospital pharmacy environment required.
Computer skills are highly desirable, especially in the application of word processing, spreadsheet, and database creation and management
Preferred Skills and Qualifications
- Knowledge of psychotropic medications and experience working in a psychiatric facility/unit
- Completion of a Pharmacy Residency program (PGY1/PGY2)
- Completion of Psychiatric Pharmacy Residency Program (PGY2)
- Board Certification as a Specialist (i.e. BCPS, BCCP, BCNSP, BCOP)
