The Program Assistant- Population Health Coordinates and implements outreach and educational material via multiple modalities to support ongoing population health and HCC projects. Provides support for data entry and maintenance of data bases. Manages/supports variety projects simultaneously. Researches and analyzes data. Population Health main function is to support the providers in managing their patient populations related to chronic disease states and preventative care measures. Team size is 2-3 people.

Sharp Community Medical Group (SCMG) is one of the state's largest and most successful IPAs. Established in 1989, it has over 140,000 members and is comprised of 200 primary care physicians and 500 specialists. The Health Services division of SCMG coordinates the medical care of members for inpatient and outpatient services and has a staff of 80 nurses, coordinators and support personnel to manage utilization review, case management and quality improvement functions. Sharp Community doctors admit to all Sharp hospitals and Rady Children’s Hospital. In addition, Sharp Community patients have access to urgent care centers throughout the county.


This position is located in Kearny Mesa, San Diego County



8 hour day shift, 8am-5pm; Weekends as needed


Required Skills and Qualifications

  • Five (5) years experience in position requiring significant levels of cooperation, collaboration, analysis, troubleshooting, and problem resolution
  • Previous experience as a medical assistant
  • Experience with coding ( CPT & ICD-9/10)
  • Experience working with Excel / spreadsheets
  • Experience working on an EHR
  • Strong oral, written, and editing communication skills, strong computer skills, and effective interpersonal skills    


Preferred Skills and Qualifications

  • Bachelor’s degree in health care or business
  • HCC coding experience
