This position will work with colleagues in the social services department, MD's and the treatment teams to pursue appropriate discharge plans for the patient and family. Contributes to the development of a comprehensive treatment plan by actively communicating and sharing information with other members of the treatment team. Provides direct discharge planning services and medical record documentation to patients and families. May conduct some groups. This positions will work primarily as a Social Worker/ Discharge Planner covering locked and open units as necessary in a psychiatric hospital.
This is a very fast paced position with a tremendous opportunity for professional growth within a solid professional Social Services department. The Social Services Department provides a wide range of services to patients and their families. The department consists of Social Workers, Discharge Planners, Chemical Dependency Counselors, a post-doctoral psychology fellow, and social work interns. Social Workers are part of the treatment team and work in collaboration with nurses, psychiatrists, pharmacists, recreational therapists and dieticians to ensure that every patient receives the best care possible.

Per Diem; Hours will vary as needed. 8-hour shift; 8:00 am - 4:30 pm, weekend as needed
Required Skills and Qualifications
- Bachelor’s in Social Work
- Acute/psychiatric hospital experience
- Knowledge of Discharge planning resources and referral processes
- Knowledge of medical/psychiatric disorders, chemical dependency/substance abuse, and basic psychopharmacology
- Knowledge of local community resources
- Effective interpersonal and communication skills, skills in group facilitation
- Experience working in a hospital setting as a discharge planner/patient navigator
Preferred Skills and Qualifications
- Three years of experience in a behavioral health setting
- Medical/Behavioral Health/Discharge Planning experience
- Bilingual (English/Spanish)
