What You Will Do
To provide documentation for discharge planning, psychosocial, treatment plan, UR review, and care coordination for patients. Intake phone calls and assessment patient for appropriate programs.
Required Qualifications
- Bachelor's Degree In Social Work.
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) -REQUIRED
Preferred Qualifications
- Hospital experience.
- Case management experience.
