$43.567 - $56.215 - $68.864The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.
What You Will Do
To provide consultation and psychosocial counseling and support services to patients and their families. At least 50% to 70% of the time is clinical time. Provides leadership and coordination of the operations of the social work department.
Required Qualifications
- Master's Degree
- 5 Years clinical experience, preferably in a health care setting.
- 2 Years clinical experience in an inpatient setting.
- California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -REQUIRED
Preferred Qualifications
- Supervisor experience.
Essential Functions
- Achieves operational outcomes
Identifies and seeks resolution to operational flow issues and performs one or more of the following:
Collaborates with other disciplines/departments/physicians to resolve identified issues; and/or
Establishes linkages between the various Sharp entities/SW Leads that results in the integration of policy, procedures, work regulations, and staffing; and/or
Facilitates staff in prioritizing and problem solving daily operational issues; and/or
Demonstrates resolution of operational issue with targeted outcomes as negotiated with manager or director. - Assess the psychosocial needs
Completes psychosocial assessments.
Identifies the key clinical issues such as grief and loss, adjustment to illness, psychiatric concerns with attention paid to the developmental milestones and life transitions that can impact functioning. - Communication
Documents assessment finding within 24 hours of referral.
Consult with team members in development of treatment plans
Maintains ongoing communication with multi-disciplinary team members regarding patient status and follow-up.
Documents ongoing progress with patients/families in the appropriate section of the medical record as required by policy and regulatory agencies.
Attends rounds in major practice areas.
Documents consultation, recommendations, interventions and treatment plans.
Identifies and communicates needs for patient care conferences.
Arranges / participates in patient care conference for difficult/high-risk psychosocial cases to enhance resolution of identified issues and promotes understanding of patient. - Community resource
Utilizes and refers to internal resources of hospital that can assist patients and families.
Refers basic community resources.
Facilitates the timely discharge of patients.
Provides referrals to patient/family for ongoing assistance after leaving hospital. - Consultation and support
Consults and intervenes in crisis situations.
Assists staff in de-escalating volatile situations.
Educates other staff in handling Code Green situations.
Coordinates and facilitates critical incident debriefings
Develops nursing education to increase staff knowledge. - Department outcomes
Participates in quality improvement activities by evaluating delivery of services and programs and performs one or more of the following:
Consults with all social workers regarding cases, policy issues or interdisciplinary matters on which important decisions need to be made; and/or
Implements systems to inform/monitor staff compliance with regulatory and accreditation standards; and/or
Implements action plans to improve clinical care of patients. - Human resource management
All 90 day and annual performance reviews are completed within 30 days of due date.
Assures employee files are current and complete, including annual PPD, CPR, safety test, etc; and performs one or more of the following:
Orients/mentors staff into new roles resulting in achieved competencies within identified timelines; and/or
Develops staff's leadership skills, facilitating learning and growth; and/or
Assists staff to resolve identified learning/performance issues; and/or
Develops new area of expertise relevant to human resource management. - Oversee productivity
Achieves productivity score between 98-100%.
Contributes to meeting budgeted salary expense per unit of service.
Coordinates day-to-day staffing activities, including scheduling, caseload assignments and census flexing.
Contributes to utilization of supplies in cost-effective manner that supports delivery of care with budgeted supply expense per unit of service. - Patient advocate
Encourages patient and family participation in the decision making process.
Reports all mandatory protective concerns such as Elder Abuse, Child Abuse and Domestic Violence as required by law.
Protects pt privacy by maintaining confidentiality.
Keeps current with regulatory and legal requirements regarding pt privacy and confidentiality standards.
Identifies bio-ethical dilemmas and initiates appropriate response - Patient goals and intervention
Develops and implements treatment goals which address identified needs.
Practices appropriate boundary setting with clients.
Demonstrates sensitivity to cultural and ethnic diversity.
Effective at crisis intervention and problem solving techniques and is a resource to others in these areas.
Seeks resources when faced with unusual or new situations. - Plan of care
Evaluates pt/family response to interventions.
Revises care provided to patients/families.
