Hourly Pay Range (Minimum - Midpoint - Maximum):

$32.730 - $40.910 - $45.810


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do
To provide consultation and management of patients with high risk and complex dual medical and behavioral diagnosis, psychosocial counseling and support services for the Sharp Community Medical Group population. To facilitate the integration and continuity of care for medical and behavioral components of patient care through effective communication, seamless care coordination through effective navigation, assisting members understand BH benefits, available programs, SHC system and community resources. To mobilize patients and families' personal and environmental resources to receive maximum benefits for both medical and behavioral health care and achieve the fullest personal and family functioning, and optimally enhance the duration of state of wellness and overall quality of life.

Required Qualifications
  • Bachelor's Degree in Social Work or other behavioral health related field.

Preferred Qualifications
  • Behavioral Health experience.
  • California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -PREFERRED

Essential Functions
  • Assess Psychosocial Needs
    Completes psychosocial assessments.
    Identifies the key clinical issues such as grief and loss, adjustment to illness, substance abuse, psycho-social behavioral response, psychiatric concerns with attention paid to the developmental milestones and life transitions that can impact functioning.
  • Communication
    Documents assessment finding per department guidelines.
    Consult with PCP, BH providers and other multidisciplinary team members in development of case management plans.
    Maintains ongoing communication with multi-disciplinary team members regarding patient status and follow-up.
    Documents ongoing progress with patients/families in the appropriate patient case file as required by policy and regulatory agencies.
    Attends case management review as appropriate based on department standards.
    Documents consultation, recommendations, interventions and case management care plans.
    Identifies and communicates needs for patient care conferences.
    Arranges / participates in patient care conference for difficult/high-risk psychosocial cases to enhance resolution of identified issues and promotes understanding of patient.
  • Community Resource
    Utilizes and refers to internal resources of hospital and community resources that can assist patients and families.
    Facilitates the timely transition of patient between at every point of the care continuum, ER, inpatient medical/behavioral, outpatient care centers, alternative living provisions, B/C or SNF.
    Provides referral to patients and families for ongoing community services/support.
  • Consultation and Support
    Consults and intervenes in crisis situations.
    Assists staff in de-escalating volatile situations.
    Educates and consults with other staff in handling psychosocial crises situations.
    Coordinates and facilitates critical incident debriefings.
    Develops nursing education to increase staff knowledge.
  • Patient Advocate
    Encourages patient and family participation in the decision making process.
    Reports all mandatory protective concerns such as Elder Abuse, Child Abuse and Domestic Violence as required by law.
    Adherence to all mandatory and regulatory requirements specific to Behavioral Health management of communication, patient information (HIPPA) and protection of specific conditions/diagnosis.
    Protects patient privacy by maintaining confidentiality.
    Keeps current with regulatory and legal requirements regarding patient privacy and confidentiality standards.
    Identifies bio-ethical dilemmas and initiates appropriate response
  • Patient Goals and Intervention
    Develops and implements care management goals based on medical and behavioral health plan of care which address identified needs.
    Practices appropriate boundary setting with clients.
    Demonstrates sensitivity to cultural and ethnic diversity.
    Effective at crisis intervention and problem solving techniques and is a resource to other case management and multidisciplinary care team members.
    Seeks resources when faced with unusual or new situations.
    Utilizes appropriate treatment modalities including individual, conjoint, and group.
  • Plan of Care
    Evaluates patient/family response to interventions.
    Revises care management plan provided to patients/families.

  • Theoretical and practical knowledge of psychosocial process, strong assessment and clinical counseling skills.
  • Excellent communications - oral and written.
  • Excellent problem solving skills.
  • Excellent documentation skills.
  • Strong collaborative and negotiation skills.
