$23.000 - $23.000 - $26.777This position is covered by a Collective Bargaining Agreement (CBA) with SEIU-UHW. As part of the terms of employment, employees in this role are required to join the union within 31 days of hire and remain a member (e.g. dues paying, fee paying, religious exception contributor) for the duration of the collective bargaining agreement.
What You Will Do
To develop, organize, promote and lead daily therapeutic recreational programs/activities with resident groups in the SNF setting in consideration of resident's age, physical ability, mental functioning and interests.
Required Qualifications
- H.S. Diploma or Equivalent
- AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED
Preferred Qualifications
- Bachelor's Degree
- 2 Years experience in social or recreational program (within last 5 years).
- 1 Year patient activities program in a health care setting.
Essential Functions
- Assures availability of patient equipment and supplies
Provides patients with appropriate activity equipment or supplies as needed.
Checks, cleans equipment and supplies as appropriate as indicated by Infection Control Policy.
Checks and follows up on patient's request for additional or change in supplies or activity equipment.
Utilizes supplies in a cost effective manner.
Obtains and returns supplies and materials as appropriate. - Completes or assists with activity programs
Assists in the development of master calendar of events.
Helps identify resources, guests who will be valuable for activity completion/implementation.
Displays the monthly calendar in designated areas of the unit.
Leads/and or assists in group activities using various forms of equipment/supplies.
Transports patients to and from activities as needed.
Assists in the development and implementation of room activities for residents who require one-on-one attention.
Identifies patient's recreational needs and encourages them to attend/participate.
Assists and or conducts initial ongoing assessment on each resident per designated time frame.
Obtains information from (but not limited to) resident, family, and medical chart.
Develops care plans/goals using information obtained from the initial and ongoing assessment.
Consults with the IDT Team to further assess resident's needs and preferences. - Patient care activities
Completes and signs initial and ongoing activity assessment accurately and timely.
Identifies and communicates new resident/patient care issues/concerns.
Communicates changes in patient's/resident's condition.
Documents daily attendance record of patient's activities.
Writes care plans as prescribed by identified problems/issues and evaluates outcomes.
Communicates incomplete work requiring follow-up to Activity Coordinator. - Time management
Demonstrates ability to prioritize workload in accordance with patient's needs and staffing pattern, as appropriate.
Validates understanding of new patient needs or changes in assignments.
Communicates with other staff to develop plan for best use of resources.
Asks for assistance to get workload done/completed.
Reports to Activity Coordinator when unable to respond to number of current demands.
Seeks feedback from co-workers about strategy to complete work more efficiently.
