Hourly Pay Range (Minimum - Midpoint - Maximum):

$39.435 - $50.884 - $62.333


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do
Provide individual family and group counseling to hospice survivors for up to thirteen months following the death of a hospice patient. Provide information/education related to the grief process to the survivor and community referrals as needed. Consultation regarding the bereavement program or aspects of the grief process may also be provided to the community.

Required Qualifications

  • Master's Degree In social work, counseling or psychology (MSW, MFT or MA).
  • 2 Years Counseling experience.
  • Driver's License - CA Department of Motor Vehicles -REQUIRED
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED


Preferred Qualifications

  • Hospice and bereavement experience.
  • California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -PREFERRED
  • California BBS Licensed Marriage and Family Therapist (LMFT) - CA Board of Behavioral Sciences -PREFERRED


Other Qualification Requirements

  • Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.


Essential Functions

  • Assessments
    Completes comprehensive bereavement assessments within specified time frame from referring discipline and assesses needs throughout time on service including, but not limited to:
    General Coping
    Outside support
    Safety issues
    Outside community providers
    Grief symptoms
    Mental / physical status
    Bereavement risks
  • Caseload
    Sets daily priorities and revises schedule based on patient / family needs.
    Responds to crisis and issues in a timely manner.
    Maintains contact with patient and families based on department guidelines, need and plan of care.
  • Clinical skills
    Provides counseling / education related to coping and grief.
    Provides counseling / education / interventions related to urgent safety issues.
    Provides counseling / education r/t new problems and issues identified.
    Provides interventions to client to assist with coping.
    Able to facilitate groups and provide counseling services in large group dynamics.
    Provides client centered care and promotes autonomy and client advocacy.
    Able to identify needs for additional community resources and able to refer appropriately.
  • Documentation
    The clinician is able to document professionally and appropriately in regards to, but not limited to: Initial assessments, routine visits, group work, clinical communication with team, and discharges.
    Additionally, documentation is completed by end of shift and is individualized per patient.
  • Plan of care
    Develops and revises plan of care based on client needs.
    Performs services as ordered.
    Documents according to plan of care.
    Documents changes in plan of care.
    Follows visit frequency and updates based on client need and clinical judgement.
  • Professional responsibilities
    Identifies and clarifies professional role and performs within scope of practice.
    Demonstrates respectful and professional behavior with employees, patients, families and community partners.
    Sensitive to cultural differences and practices culturally competent care.
