NoHourly Pay Range (Minimum - Midpoint - Maximum):
$24.500 - $29.400 - $34.300This position is covered by a Collective Bargaining Agreement (CBA) with SEIU-UHW. As part of the terms of employment, employees in this role are required to join the union within 31 days of hire and remain a member (e.g. dues paying, fee paying, religious exception contributor) for the duration of the collective bargaining agreement.
What You Will Do
Performs as the coordinator of communication for the unit and assumes primary responsibility for medical record management. Greets visitors and ensure meeting of needs or referred to appropriate resources. Facilitates patient data management through transcription, order entry, and documentation. Support best use of resources in management of equipment, supplies, and other department resources.
Preferred Qualifications
- H.S. Diploma or Equivalent
- 1 Year Experience as Unit clerk/secretary in an acute care setting
Essential Functions
- Admissions, discharges and transfers
Enters accurate disposition of patient requiring admission, transfer or discharge per unit time guidelines.
Requests medical records, films, or other medical record information from other facilities.
Coordinates scheduling of tests and retrieving results to support patient progress.
Collects and prepares patient data necessary to support interfacility transfer.
Dismantles chart and returns to Medical Records, including old charts. - Customer service and communication
Greets and makes welcome a variety of customers by the telephone and in person.
Obtains information from visitors/callers, directs as appropriate or takes complete and accurate messages. Acts on requests for customer assistance.
Uses scripts as appropriate including answering phones, transferring calls and service recovery.
Answers call lights within 4 rings or calls into patient rooms to validate request.
Answers phone within 4 rings; transfer calls as indicated.
Responds to all inquiries with a timeliness that promotes customer satisfaction.
Coordinates with physicians and departments team members via paging system or phones.
Assists in directing others during crisis interventions (codes, disasters, etc.).
Communicates effectively with all levels of staff and uses chain of command per unit guidelines. - Data entry and inquiry
Accurately enters orders in electronic information systems, noting order number within 2 hours of availability to clerk.
Enters stat order within 30 minutes and communicates with appropriate staff.
Seeks clarity before completing the entry when order not clear or refers to nurse for follow-up.
Retrieves diagnostic study, lab results, and other relevant patient information supportive of patient assessment.
Completes order entry via down time procedures when electronic information systems not available. - Department efficiency and effectiveness
Efficient in completing own work, organizing, prioritizing and delegating to support productive completion of work within unit time guidelines.
Orders supplies, forms and equipment, maintains stock per guidelines.
Manages equipment through proper operation, obtaining supplies, troubleshooting and calling for maintenance (e.g. fax, copier, printer, tube system, phone systems, intercom and other equipment supportive of completing clerical work).
Logs, assigns, and tracks wireless phones and other portable equipment.
Enters patient charges into computer systems without duplication.
Supportive of efficient management of other department resources (e.g. keys, signage, other) per guidelines.
Evaluation Method(s):
O, D, QM, T, DR, TD - Medical records management
Obtains chart inserts for admissions and maintains charts, insuring additional forms are available and placed in charts.
Obtains old charts from medical records.
Requests copies of H&P, consults, or other paper reports to complete medical record.
Demonstrates patient's 24 hour charging record and place record in patient's chart.
Flags charts for physician signatures on verbal orders.
Files interdisciplinary and diagnostic printouts in chart per standards.
Dates, times, and initials all entries.
Assists team members in locating patient data information. - Transcription
Transcribes routine orders within 2 hours of availability to clerk.
Transcribes stat orders within 30 minutes and communicates with appropriate staff.
Seeks clarity before completing the transcription when order is not clear or refers to nurse for follow-up.
