$64.955 - $83.813 - $102.671


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do
The Sr. Specialist Medical Social Work (SW) leads advanced psychosocial and social drivers of health (SDoH) clinical interventions and care planning to achieve safe and sustainable care transitions for Sharp patients considered the most complex and/or vulnerable as assigned. In addition, this position serves as a resource for other medical social workers and members of the interdisciplinary care team (IDT). The Sr. Specialist Medical Social Work is the clinical champion and works in close coordination with the Manager of Social Work, site leadership and System ICM Support Services. The Sr. Specialist will consult on and may assume ownership of escalated cases as determined by site ICM Leadership. This role will mentor and provide oversight for social work MSW students and provide support to SW's seeking LCSW licensure. This position acts as a subject matter expert on complex care assessment and planning that extend beyond standard practice, considering all extraordinary medical, behavioral, psychosocial as well as any social drivers of health concurrently intersecting with the index need. The Sr. Specialist Medical Social Work takes a principle role in identifying education gaps and needs and partners with the System ICM Clinical Educators to create, implement and monitor for education adherence and sustainment. This position may be assigned projects to drive and monitor change to ensure departmental key performance metrics are met. The Sr. Specialist will aid in department operations, including but not limited to, new team member orientation, education support, report development, data collection and scheduling. The Sr. Specialist is a key member of the site Utilization Management Committee.

Required Qualifications
  • Master's Degree in Social Work.
  • 3 Years acute care or clinical experience in area of specialty.
  • California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences -REQUIRED

Preferred Qualifications
  • Accredited Case Manager (ACM) - American Case Management Association (ACMA) -PREFERRED
  • Certified Case Manager (CCM) - Commission for Case Manager Certification -PREFERRED

Other Qualification Requirements
  • Accredited Case Manager (ACM) - American Case Management Association (ACMA) OR Certified Case Manager (CCM) - Commission for Case Manager Certification - Required within 1 years of hire. Department will track and maintain this certification.

    Establish effective working relationships between internal and external customers with a focus on efficiency and superior outcomes.
    Partner with System and Regional/Metro Market ICM site leadership to ensure timely communication and facilitation of communications to ensure optimal operations.
    Deliver answers and solutions within the agreed upon timeframe.

    Seek to improve systems and processes that ultimately improve staff performance and assist in reducing the overall cost of care in collaboration with other members of the ICM Leadership team.

    Support department operations as directed.

    Serves as an informal member of the ICM Leadership team.
    Actively participate in ICM team and leadership meetings.

    Works with the SW team members to achieve departmental and System goals.

    Establish mutually derived annual goals and meets goals.
    Maintain individual in-service/performance records.
    Ongoing professional growth through attendance at educational forums as identified to support the roles and responsibilities of this position.
  • Quality and Safety
    Partners with the System ICM Quality & Regulatory Specialists and System ICM Educators to drive change and standardization. Takes audit findings and works with identified team members to create action plans and reports back to the System ICM and Site leadership.
    Identify, drives and/or participates in initiatives to improve work processes with focus on superior outcomes.
    Actively practices safe work habits and contribute to ensuring a safe work environment.
    Utilizes appropriate processes or tools to document identified problems.
    Make sound decisions and demonstrates ability to handle complex situations not covered by written or verbal instructions.
    Escalate cases and situations to the appropriate resources timely when issues or problems arise.
  • Clinical Competency

    Accredited Case Manager (ACM) - American Case Management Association (ACMA) OR Certified Case Manager (CCM) - Commission for Case Manager Certification - Required within 1 years of hire. Department will track and maintain this certification.
