Hourly Pay Range (Minimum - Midpoint - Maximum):

$56.580 - $70.170 - $83.760


The stated pay scale reflects the range as defined by the collective bargaining agreement between Sharp HealthCare and Sharp Professional Nurses Network, United Nurses Associations of California/Union of Health Care Professionals, NUHHCE, AFSME, AFL-CIO.  Placement within the range is based on years of RN experience.

This position is an exciting new addition to the Sharp Grossmont Integrated Care Management Department and is field based with no hospital direct patient care or discharge planning requirements. Public Health RN, case finding, data collection and excellent communication skills preferred.


What You Will Do
The Care Transition Coach - RN is a licensed Registered Nurse who empowers the patient and/or family/caregiver to play an active and informed role in their health care. The Care Transition Coach - RN helps the patient to become good at and comfortable with, managing their care after hospital discharge. The Care Transition Coach - RN provides guidance to the patient for effective care transitions after hospital discharge, improving self-management skills and enhancing patient-provider communication. The Care Transition Coach - RN also facilitates interdisciplinary collaboration and care continuity across care settings. The Care Transition Coach - RN does not provide direct patient care or treatment.



Required Qualifications

  • Associate's Degree in Nursing Or Bachelor's Degree in Nursing
  • 2 Years experience as a program manager/coordinator and/or case manager which includes developing a program from concept to implementation to evaluation.
  • 5 Years clinical/nursing experience.
  • Driver's License - CA Department of Motor Vehicles -REQUIRED
  • California Registered Nurse (RN) - CA Board of Registered Nursing -REQUIRED


Preferred Qualifications

  • Prior public health experience - strongly preferred
  • Experience with discharge planning, home care or similar acute to post-acute transitions role
  • Experience working with patients with chronic or debilitating health conditions requiring hospitalization (e.g., CHF, Diabetes, COPD, Pneumonia, AMI)


Other Qualification Requirements

  • Certification in Care Transitions Intervention® (CTI) Coaching (required within 60 days of obtaining the position).

    Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.

    Resolves educational deficits.
    Develops new area of expertise/skill acquisition applicable to role.
  • Community and leadership
    Demonstrates commitment to professional growth and contribution to profession and/or community health.
    Develops and conducts community-based education about profession/specialty area.
    Member of professional organization related to specialty.
  • Educator
    Develops framework for assigned clinical staff competency validation and provides opportunities for professional growth.
    Develops framework for patient/family education in selected patient populations.
    Develops and maintains department specific competency program.
    Educates staff on new or advanced application of skills.


Knowledge, Skills, and Abilities

  • Bilingual English/Spanish strongly preferred.
  • Proof of insurance required - must be able to travel throughout San Diego County.
